We require a copy of your SAR with your scholarship application.
The SAR is the form you receive after you file your FAFSA and is usually
5 pages. We require page 1 and the pages that include your
responses to the FAFSA questions. These are usually on pages 2 and
3. This year, the SAR is titled "Federal Student Aid FAFSA" and
the form is OMB No.1845-0001 as shown on the upper right corner.
(If your form shows a different OMB number, please contact us.)
Please note that your name is not on page 1. It would be helpful
to us if you printed your name at the top of page 1.
If you need to make corrections to your FAFSA,
enclose the SAR you receive after the corrections have been completed.
If FAFSA states that your application has been selected for
verification, you may still submit this SAR with your scholarship
application. (Your school may require more documentation.)
